To get a Swasthya Sathi card, you need to apply offline at the municipality or panchayat office or at the door of the government camp in your area.
You need to download and print the required application form online, then fill it properly, attach the required documents with Xerox and submit it to the local municipality or panchayat office.

Step 1: Visit the official website of the Swasthya Sathi Scheme First, you need to go to the official website of the Health care Scheme.
Then click on “Find Your Name” in the menu section above. If you are opening from mobile then you will get this “Find Your Name” option by clicking on the menu.
Step 2: Enter the mobile number Now enter the mobile number that you gave at the time of application.
If you do not remember the number you entered, enter your primary mobile number. Now select your own or others’ names from the options. Then click on the “Submit” button. A new page will open.
Step 3: Enter Aadhaar / Ration Details Choose state name, district name, block/municipality, and other details. In the “Select by” box, select documents submitted during the Health care Card application.
Enter the document number. Now click on submit button. Your name, status, and other details related to your health care card will appear on your screen.
